• The right you wish to exercise (access, correction, deletion, portability, opt-out, restriction)
• The email address associated with any account or prior correspondence
• Enough context for us to identify the records (e.g. approximate dates, services used)

We acknowledge requests within 3 business days and provide a substantive response within 30 days, per our Privacy Policy.
